The Public Health Alliance of Southern California is founded on the idea that everyone deserves the opportunity to live a full and healthy life, regardless of who they are, where they were born, or where they live. In partnership with the County of San Diego, we designed a brief overview document to highlight the benefits of the Farmer’s Food as Medicine program, which provides community members with 12-months of home delivered farm-fresh produce boxes, to be followed by 6-months of CalAIM Medically Tailored Meals.
